The Form was developed to provide a detailed, rapid, and qualitative description of the physical, chemical, and biological characteristics of wetlands being evaluated under the Clean Water Act (CWA) Section 404 permitting process. The Form will provide additional supporting documentation regarding the condition of areas proposed to be impacted and assist the Norfolk District in making more informed permit decisions. This Form does not take the place of a function/values assessment. The Form is not to be used for compensatory mitigation. We highly recommend applicants complete the Form as a component of all permit applications that propose to impact an acre (1) or more of wetlands.
